Questionnaire: Is Your Currnet Parkinson's Disease Therapy “Wearing-Off”? After taking a dose of levodopa, most people with Parkinson's disease experience a noticeable improvement in their symptoms. Over time, however, many patients notice that their symptoms return before it's time for their next dose. This is called "wearing-off." Symptoms of wearing-off include slowness of movement, tremor, and muscle cramping.

To inform your healthcare professional if you are experiencing any symptoms of wearing-off, please complete this questionnaire, print it out, and return it to him or her.

Please check in Column A any symptoms that you currently experience in your normal day. Please also check the box in Column B if this symptom usually improves or disappears after you take your next dose of your Parkinson's medication.
